What to Do When the Tooth Fairy Isn’t on Schedule

August 22 is National Tooth Fairy Day! If the Tooth Fairy is a treasured part of your child’s life, you’re just in time to celebrate! But what to do when the Tooth Fairy doesn’t arrive on schedule—when baby teeth stay longer than expected or are lost too soon? 

A baby’s 20 baby teeth tend to come in within a fairly predictable time frame. The bottom central incisors in the very front of your baby’s mouth typically make their appearance first, when your baby is around six to ten months old. Over the next few years, the remaining incisors, canines, and first molars arrive. Last on the scene are the second molars, which usually show up between the ages of 23-33 months. 

Just as baby teeth follow a pattern coming in, they tend to follow the same pattern falling out. The front teeth begin to wiggle and loosen around age six or seven, while the last of the baby teeth, the canines and second molars, are often lost between the ages of ten and 12. 

Baby teeth fall out as the adult teeth below them push up as they erupt. The top of the new tooth puts pressure on the root of the baby tooth, gradually dissolving it. As the root grows smaller and can’t anchor the tooth, the tooth begins to wiggle and eventually becomes loose enough to fall out. This leaves the adult tooth perfectly placed to grow into its proper position.

Sometimes, though, teeth linger far past their fall-out date. Sometimes, because of decay or trauma, they are lost much too early. In either case, Dr. Meuselbach can provide treatment to protect little smiles now and to ensure that there’s space for the permanent teeth to erupt and align correctly.

Teeth Which Overstay Their Welcome

If that baby tooth never gets wiggly, the team at Robert Scott Meuselbach DDS can help! When baby teeth stubbornly hang on, adult teeth can erupt behind them, creating a double row of teeth commonly known as “shark teeth.” These permanent teeth can become crowded or misaligned as they try to fit in any space available. Or a baby tooth can block an adult tooth from erupting at all. When that baby tooth just isn’t budging, an extraction will create space for the permanent tooth to erupt.

Extracting a baby tooth is generally a straightforward procedure because primary teeth have very small roots. Your child’s dental team at Robert Scott Meuselbach DDS are experts in helping you prepare your child for the procedure in a gentle, reassuring, and age-appropriate way. 

Often, a local anesthetic is all that’s necessary for a simple extraction, but if you feel sedation would better fit your child’s needs, discuss sedation options with your dentist. After the extraction, you’ll be given clear information on how to deal with pain and swelling, which foods and drinks are best while the extraction site heals, and how to protect the area.

Teeth Which Exit Too Early

In the case of decay or trauma, Dr. Meuselbach will do everything possible to save the tooth. When decay is so extensive that there’s not enough structure left to hold a filling or crown, or when there’s an infection in or around the tooth, or when an accident or injury has caused serious damage, extraction might be the healthiest option. Depending on your child’s age, further treatment might be needed afterward to protect future smiles. 

Besides their roles in eating and speaking, baby teeth save space for permanent teeth. Whenever a child loses a primary tooth too soon, the family dentist will be on the lookout for potential future orthodontic problems. If remaining baby teeth shift, taking up part of the empty space left behind by the lost tooth, the adult teeth below won’t have the space they need to erupt properly. Permanent teeth might come in at an awkward angle or in the wrong place. 

In this case, Dr. Meuselbach might recommend a space maintainer. Space maintainers are small, custom-designed appliances which prevent the remaining baby teeth from shifting position while holding space open for the permanent tooth to arrive right on schedule and right where it belongs. 

It’s comforting to have charts which let us know when little teeth will typically come in and when they will typically fall out. But unexpected events might mean teeth overstay their welcome or depart too soon. Is There an Act Three for Our Teeth?

Act One: Those 20 adorable baby teeth which begin arriving when you’re about six months old and are probably gone by the time you’re twelve.

Act Two: The 32 (if you have all your wisdom teeth!) adult teeth which start showing up around the age of six, and need to last the rest of your lifetime. Unless there’s an . . .

Act Three: Where science has discovered a way to repair damaged teeth or create replacements for lost teeth that would look and function just like our original teeth.

Sounds like science fiction, doesn’t it?

In biological terms, of course, we’re not there yet. But teams of scientists around the world have been working toward just this goal—the ability to repair a tooth with its own biological material or even grow a new tooth when an adult tooth is seriously damaged or lost.

Because the pulp—the living tissue inside each tooth—contains dental stem cells, some researchers are studying whether these cells can be used to regenerate the living pulp inside a damaged tooth. Even more exciting, there’s research being done into the possibility of using these stem cells to rebuild an entire tooth—pulp, dentin, and hard enamel exterior. Because the supply of usable dental stem calls is quite small, and process of guiding these cells to create all the varied parts of the tooth is quite complex, this research is in its early stages.

In other studies, scientists have focused on tooth regeneration by studying the teeth of other species. Mice, for example, have front teeth with open roots. This allows these teeth to keep growing up as they wear down, because stem cells inside mouse gums are always working to create new tissue. Even though our teeth are not exactly comparable (no open roots in human teeth!), finding out just how this tissue regeneration works in mice and other animals might help find an answer for human tooth regeneration.

Right now, these scientific goals are just that—goals for a future day. But in the meantime, your quality of life doesn’t need to suffer when you lose an adult tooth. You can enjoy a third act right now with dental implants.

Cosmetic concerns aren’t the only reasons why you want your complete smile restored in the event of a lost tooth. A missing tooth can lead to many unpleasant consequences:

  • Speaking and eating can become more difficult.
  • Lack of stimulation from biting and chewing can cause the bone beneath the lost tooth to shrink.
  • Other teeth can shift to fill the gap left behind, which can affect tooth, bite, and jaw alignment.
  • Loss of teeth can lead to loss of confidence.

The Link Between HPV and Oral Cancer

Cancer has become a common word, and it seems like there is new research about it every day. We know antioxidants are important. We know some cancers are more treatable than others. We know some lifestyles and habits contribute to our cancer risk.

Smoking increases our risk of cancer, as does walking through a radioactive power plant. But there is a direct link to oral cancer that you many may not know about—the link between HPV (Human Papilloma Virus) and oral cancer.

This may come as a shock because it has been almost a taboo subject for some time. A person with HPV is at an extremely high risk of developing oral cancer. In fact, smoking is now second to HPV in causing oral cancer!

According to the Oral Cancer Foundation, “The human papilloma virus, particularly version 16, has now been shown to be sexually transmitted between partners, and is conclusively implicated in the increasing incidence of young non-smoking oral cancer patients. This is the same virus that is the causative agent, along with other versions of the virus, in more than 90% of all cervical cancers. It is the foundation's belief, based on recent revelations in peer reviewed published data in the last few years, that in people under the age of 50, HPV16 may even be replacing tobacco as the primary causative agent in the initiation of the disease process.” [http://www.oralcancerfoundation.org/facts/]

There is a test and a vaccine for HPV; please discuss it with your physician.

There are some devices that help detect oral cancer in its earliest forms. We all know that the survival rate for someone with cancer depends greatly on what stage the cancer is diagnosed. Talk to Dr. Meuselbach if you have any concerns.

Oral Health Concerns for Teens

You have a lot more freedom as a teenager than you did as a young child. You also have a lot more responsibilities, and one of your jobs is to take care of your teeth. Develop and maintain good dental habits now so you can have great dental health for life!

Tooth Decay

As a teenager, you risk tooth decay, or dental cavities, if you are not careful. In fact, 59% of adolescents aged 12 to 19 have at least one cavity, according to the National Institute of Dental and Craniofacial Research. Dr. Meuselbach and our staff recommend keeping your teeth strong and healthy by brushing at least twice a day and flossing every day.

If you suspect that you have tooth decay, do not be embarrassed. Instead, ask your parents to bring you to Robert Scott Meuselbach DDS to get it looked at. When you do not treat your dental cavities, they can turn into more serious problems. A severely damaged tooth may need to be treated with a root canal or even an extraction.

You can take easy steps to prevent tooth decay when you are at school or hanging out with your friends. Carry a bottle of water around with you so you can take a sip after you eat any kind of food. Choose water or milk instead of soda or sports drinks, and if you chew gum, select a sugar-free flavor.

Other Oral Health Concerns

You can probably think of many reasons why you should not smoke or use tobacco. Your oral health is another one. Tobacco gives you bad breath and stains your teeth yellow. It also increases your risk for gum disease and cancer of the mouth. Smoking even slows the speed of healing after you have dental procedures done.

Here are a few more tips that can keep your mouth attractive and healthy during your teen years.

  • Drink plenty of milk.
  • Limit candies and sugary snacks.
  • Wear a mouthguard if you play a contact sport.

  • Reduce infections and avoid piercing your tongue and lips.
